Register for a Library Card

Welcome to the Library!

This form is to be filled out by those that are sixteen years or older. Dependents under the age of sixteen must accompany adults to the library for in-person registration, or use the Juvenile Card Registration form.

Cost of Your Library Card

Library cards are free for all residents of Madison County, Alabama. Non-residents can pay a $20 annual fee for an individual library card or $35 for the whole family.

Getting Your Library Card

Please visit any branch of the Huntsville-Madison County Public Library within the next 30 days to validate your registration and receive your new library card. 

In completing and submitting this form, I understand that I am applying to receive library privileges for use at all branches of the Huntsville-Madison County Public Library. As the adult on this account, I commit to be responsible for my own account and for my registered dependent children. I commit to comply with all library rules and policies, and to promptly pay any fines or damages charged to me or my registered dependents. I understand that late or damage fees may be assessed to my account if items are returned late or in poor condition, and I will be charged replacement fees for items that are lost or not returned.
